• Please do NOT attend an appointment if you have:

​​Been diagnosed with Coronavirus.

Been in contact with a person diagnosed with Coronavirus.

Recently returned from an officially restricted area.

Recently returned from Lockdown areas in Italy or Special Care Zones in South Korea.

Returned from mainland China, South Korea, Hong Kong, Japan, Macau, Malasyia, Singapore, Taiwan or Thailand in the last 14 days and have a high temperature, cough or shortness of breath.

Returned from Northern Italy, Cambodia, Laos, Myanmar or Vietnam and have a high temperature, cough or shortness of breath.
